For firefox there is this addon, it has more features than what you are asking for though.

Always force 480p quality on Youtube by default.
Hi,
on youtube the video quality is always set to auto... Sometimes it goes down to 144p despite a good internet connection & It's getting annoying now.
So to fix this I need a userscript to set the video quality 480p by default instead of auto without login and change playback for your TY account in settings. Every video regardless of my connecting will always be at 480p no matter what. If the video doesn't have 480p then it will default to the low possible quality video (360p...).
Ok so that is what I want, (no more options) I hope someone find time to write an userscript for the n00b who i'm :-P
